چکیده

Glaucomatous optic neuropathy is characterized by changes in the intrapapillary and parapapillary region of the optic nerve head, including excavation of the optic nerve head and consequent defects in retinal sensitivity with visual field defects and other psychophysical alterations. Clinical evaluation of the optic nerve head has been shown to have a high specificity and good precision for glaucoma diagnosis and an experienced observer may in fact be better in distinguishing between normal and glaucomatous disks in comparison with the HRT or OCT. It is important to differentiate normal physiologic variations in the optic cup, the neural rim, and the peripapillary retina, developmental anomalies and nonglaucomatous optic atrophies from glaucomatous optic nerve head changes. This review attempts to elucidate the morphology and anatomy of the opic nerve head, and correlate the same to the pathophysiology of glaucomatous optic neuropathy.

Clinical Applications of Optical Coherence Tomography in Ophthalmology

Assessment of the peripapillary nerve fiber layer and macular thickness can be determined in ophthalmology using optical coherence tomography (OCT). Decreased nerve fiber layer thickness and macular ganglion cell thickness in optic nerve ischemia have been correlated with visual field loss. OCT allows deep optic nerve head evaluation which helps understand pathophysiology of diseases.  Furtherm...

Evaluation of the Optic Nerve Head in Glaucoma.

Glaucoma is an optic neuropathy leading to changes in the intrapaillary and parapaillary regions of the optic disk. Despite technological advances, clinical identification of optic nerve head characteristics remains the first step in diagnosis. Careful examination of the disk parameters including size, shape, neuroretinal rim shape and pallor; size of the optic cup in relation to the area of th...

Diagnostic Methods for Optic Nerve Head and Retinal Nerve Fiber Evaluation in Glaucoma

Glaucoma which is a leading cause of blindness in the world is not a single disease but a group of disorders with diverse clinical manifestations. If not controlled at an early stage, it causes irreversible damage to vision. Proper medication slows down or even halts its growth. Identifying glaucoma at a very early stage is vital and at the same time difficult. Careful evaluation of Optic nerve...

Pathophysiology of Glaucomatous Optic Neuropathy: Role of Optic Nerve Head Vascular Insufficiency

In 1970, based on my experimental and clinical studies, I defined glaucoma as: “a disease wherein the normal balance between the intraocular pressure and the blood pressure in the choroidal vessels supplying the optic disc and retrolaminar part of the optic nerve is disturbed, resulting in vascular insufficiency in the optic disc and retrolaminar part of the optic nerve, and hence in visual fie...
